Erwin P. Simanjuntak, MSN-Ed, RN, ACM-RN
Educator-Leader • Certified Care Management Coordinator • Utilization Management Review Specialist
Erwin founded EPS Health to bridge the gap between families navigating complex care transitions and the private-pay communities ready to support them.
Erwin brings a robust clinical background to his assessments, having worked extensively as a hospital nurse in inpatient acute-care units. In these roles, he provided direct, high-acuity care to adolescent and adult patients across a variety of complex clinical settings, including prestigious teaching facilities and Magnet-recognized hospitals.
Building on his bedside experience, Erwin expanded his expertise into care coordination, serving as an inpatient hospital case manager for adults and a utilization management reviewer for a private health insurance plan. Supported by a Master of Science in Nursing with an Emphasis in Nursing Education (MSN-Ed) and his board certification as an Accredited Case Manager (ACM-RN), this diverse background gives him a comprehensive understanding of the healthcare system. He intimately knows the logistical, regulatory, and clinical nuances of moving a loved one safely out of a hospital or home setting and into a new care environment.
To further advance the intersection of patient care and system innovation, Erwin is currently pursuing a Ph.D. in Nursing Science and Healthcare Leadership with a Designated Emphasis in Science and Technology Studies.
Erwin’s ultimate goal is to empower families and facilities with clinical clarity and confidence. By delivering meticulous, state-compliant documentation, he strives to capture every individual's unique needs. He collaborates closely with all care partners involved to support a smooth and dignified transition.
Outside of clinical practice, Erwin enjoys traveling to explore new states, spending time on coastal beaches, fishing, and exploring the beautiful natural landscapes of the Pacific Northwest.
